In order to submit a proper ban report, you must fill out three things.
Notice** Not all evidence that you consider sufficient will be considered sufficient during the reviewing process. A user will only be banned if it is clear as day that they were using hacks. You can post whatever evidence you want but whatever is decided by a Moderator/Admin is the final decision.
1. [Hackers Username]
2. [What The Hacker Was Doing] (Forcefield, X-Ray, Fly, etc.)
3. [YouTube Proof Video Link]
